Important Accident Benefits Update

Effective February 2017, there are some updated Automobile Accident Benefits Services (AABS) Forms including:

• Application by an Injured Person

• Application by an Insurance Company

• Response by an Injured Person

• Response by an Insurance Company

• Representing Minors and Mentally Incapable Persons

These updates will streamline the application process, as they have removed unnecessary fields, updated required signatories, and the use of plain language. Further, the following forms were also updated as of September 2016:

• Certificate of Service

• Declaration of Representative

• Notice of Motion

• Summons of Witness

• Notice of Withdrawal

• Request for an Adjournment

• Acknowledgement of Expert's Duty

These changes included updating contact information, and references made to the Rules of Practice, effective April 1, 2016. All of these forms can be found online by clicking here
